We support people across Brisbane North, Brisbane South, and the Brisbane CBD, with Toowoomba available on request. Our nurse-led approach means we help you better understand your disability and health needs, explain your NDIS plan in plain language, and connect you with the right supports at the right time. We also work closely with multidisciplinary teams so everyone involved in your care is on the same page.

Our Story
TrueCore Connect was founded by two sisters united by a deep passion for helping others and a shared journey through healthcare. With backgrounds in rehabilitation nursing, mental health, case management, and support work, we bring decades of combined experience to the heart of our services. Working side-by-side in diverse care settings, we saw firsthand the gaps in coordination and the need for truly person-centered, compassionate support—especially for individuals navigating complex health and psychosocial challenges. We knew we could do more.

With over 11 years of experience in the health industry, I bring a strong background in rehabilitation nursing and case management. Throughout my career, I’ve been dedicated to supporting patients in achieving their recovery goals and preparing for safe, effective discharge planning. Being part of each individual’s journey has been one of the most rewarding aspects of my work, and I take great pride in helping clients regain independence and confidence. My passion lies in guiding people through their recovery, ensuring they feel supported every step of the way. I look forward to continuing to serve the community and helping more individuals move forward in their health journey with care and compassion.

With over six years of experience as a Community Nurse and Family Support Worker, Amanda has developed a deep commitment to supporting individuals and families in their journey toward better health and wellbeing. Her passion for community mental health continues to grow, driving her dedication to expanding her knowledge and understanding in this vital area. Known for her compassionate nature, Amanda brings kindness, empathy, and a genuine desire to empower others to live to their fullest potential. Her work is grounded in respect, connection, and the belief that every person deserves to feel seen, heard, and supported.

Kate is a dedicated Support Coordinator with a strong background in rehabilitation nursing, bringing a wealth of clinical knowledge and genuine passion for supporting NDIS participants to thrive in the community.
With extensive experience working alongside patients to achieve meaningful rehabilitation outcomes, Kate is highly skilled in helping participants understand, navigate, and maximise their NDIS plans to continue their recovery, build independence, and progress toward their personal goals. Her approach is positive, encouraging, and empowering, ensuring participants feel confident, informed, and supported every step of the way.

Stroke recovery
Acquired Brain Injury (ABI)
Amputee's
Early onset dementia and parkinsons
Children with Autism Spectrum Disorder (ASD)

Kate is also a proud mum of two, which enhances her empathetic and family-centred approach, particularly when supporting children and families. She services Brisbane and South Brisbane, bringing a vibrant, confident, and empowering presence to our service and to the participants she supports.
